Overview

Tadalafil / Apomorphine HCl

This compounded troche pairs apomorphine's hypothalamic dopamine receptor activation with tadalafil's long-acting PDE5 inhibition. Where sildenafil-based combinations favor on-demand rapid response, the tadalafil combination is suited for patients who prefer a broader therapeutic window and the option of daily low-dose use. Apomorphine initiates central arousal regardless of peripheral vascular tone, and tadalafil sustains corporal vasodilation for up to 36 hours per dose.

Mechanism of Action

Apomorphine activates dopamine D1 and D2 receptors in the paraventricular nucleus of the hypothalamus, stimulating oxytocinergic projections to lumbosacral autonomic centers to initiate erection through the CNS independently of peripheral stimulus or vascular tone. Tadalafil is a highly selective PDE5 inhibitor with a half-life of approximately 17.5 hours, providing sustained inhibition of cGMP degradation in corporal smooth muscle. The prolonged pharmacokinetic profile of tadalafil enables both on-demand use and daily low-dose regimens. Together, apomorphine addresses the central arousal deficit and tadalafil provides a sustained peripheral vascular window, covering both etiological dimensions of erectile dysfunction.

Benefits

Central Arousal

Apomorphine activates hypothalamic dopamine pathways to initiate erection through the CNS, independent of peripheral vascular conditions or sexual stimulus alone.

Extended Window

Tadalafil's 17.5-hour half-life provides up to 36 hours of PDE5 inhibition, removing the time pressure associated with shorter-acting PDE5 inhibitors.

Daily Use Option

Tadalafil's pharmacokinetic profile supports both on-demand and daily low-dose regimens, providing flexibility not available with shorter-acting alternatives.

Dual Coverage

Central dopaminergic initiation combined with long-acting peripheral vasodilation addresses mixed-etiology erectile dysfunction more completely than either mechanism alone.

Does Tadalafil / Apomorphine HCl Troche require a prescription?

Yes. Both tadalafil and apomorphine require a valid prescription from a licensed provider.

What are the possible side effects?

Tadalafil may cause headache, back pain, myalgia, flushing, nasal congestion, and dyspepsia. Back pain and muscle aches are more commonly reported with tadalafil than other PDE5 inhibitors and typically resolve within 48 hours. Apomorphine may cause nausea, dizziness, and yawning at initiation.

Are there any drug interactions?

Nitrates are an absolute contraindication with tadalafil. CYP3A4 inhibitors including ritonavir and ketoconazole significantly increase tadalafil exposure. Alpha-blockers require cautious use and provider management given additive hypotension risk. Apomorphine combined with ondansetron or other 5-HT3 antagonists is contraindicated due to severe hypotension risk.

Is Tadalafil / Apomorphine HCl Troche safe if I have a pre-existing medical condition?

Patients with cardiovascular disease require fitness-for-activity evaluation before use. Patients with renal or hepatic impairment require dose adjustment for the tadalafil component. Not recommended in patients with hereditary degenerative retinal conditions. Patients with Parkinson's disease or active dopaminergic therapy require specialist evaluation before adding apomorphine.
