Overview

Pharmacology

RG3 (ginsenoside Rg3), NAD+, and methylcobalamin (B12) delivered via the olfactory pathway for direct central nervous system uptake. RG3 modulates neuroinflammatory signaling and supports cholinergic neurotransmission. NAD+ restores intracellular energy currency depleted by aging, oxidative stress, and neurological demand. Methylcobalamin provides the active neurological form of B12 required for myelin synthesis, one-carbon methylation, and neuronal homocysteine clearance.

Mechanism of Action

RG3 suppresses neuroinflammation via NF-kB inhibition, increases synaptic acetylcholine by blocking acetylcholinesterase, and promotes BDNF-driven neuroplasticity. NAD+ restores neuronal energy metabolism, activates sirtuin-dependent DNA repair, and reduces mitochondrial dysfunction. Methylcobalamin is the cofactor for methionine synthase, supporting the methylation cycle required for myelin basic protein synthesis, SAM-dependent neurotransmitter methylation, and homocysteine-to-methionine conversion. Elevated homocysteine is independently neurotoxic and is directly cleared by B12-dependent remethylation. All three compounds travel along olfactory nerve fibers to reach the brain within minutes.

Nasal Spray

Administered intranasally for direct delivery to the central nervous system.

Benefits

Cognitive Clarity

Enhances attention, processing speed, and working memory through improved cholinergic neurotransmission, reduced neuroinflammatory interference, and restored B12-dependent methylation in neurons.

Neuroprotection

Suppresses neuroinflammation via RG3's NF-kB inhibition, protects against oxidative neuronal death through NAD+-driven DNA repair, and clears neurotoxic homocysteine via B12-dependent remethylation.

Brain Energy

Neuronal ATP restoration via NAD+ replenishment, supported by methylcobalamin's role in mitochondrial fatty acid oxidation and the electron transport chain through succinyl-CoA synthesis.

Myelin and Axonal Integrity

Methylcobalamin is the obligate cofactor for myelin basic protein synthesis and axonal maintenance. B12 deficiency directly causes demyelination and axonal degeneration. Synapsin + B12 addresses this neurological foundation that the original Synapsin formula does not include.

Neuroplasticity

Promotes BDNF expression and synaptic remodeling through RG3, with B12-dependent one-carbon methylation supporting epigenetic regulation of neuroplasticity genes.

Mood Support

Supports dopamine and serotonin signaling through RG3's influence on monoamine neurotransmitter pathways. Methylcobalamin is required for SAM-dependent methylation of catecholamines and contributes to neurotransmitter balance and stress resilience.

Brain Longevity

Sirtuin activation, DNA repair, and mitochondrial health through NAD+, combined with B12-driven homocysteine remethylation to protect against the accelerated brain aging associated with B12 insufficiency.

FAQs

What is Synapsin B12?

Synapsin B12 is a prescription therapy used for cognitive and vitamin B12 support.

Who should use Synapsin B12?

Patients seeking support for cognitive and vitamin B12 support may qualify after evaluation by a licensed provider

Does Synapsin + B12 require a prescription?

Yes. Synapsin + B12 is a compounded nasal spray that requires a valid prescription from a licensed provider. A telehealth consultation and medical intake are required before dispensing to ensure it is appropriate for your health history, goals, and current medications.

What are the possible side effects?

Synapsin + B12 is generally well tolerated at therapeutic doses. Mild nasal irritation, congestion, or transient burning upon administration are the most commonly reported effects. Mild headache or lightheadedness may occur following initial doses and typically resolves as the body adjusts. Transient changes in sleep quality are consistent with NAD+'s influence on circadian and mitochondrial function. Methylcobalamin is not associated with toxicity at therapeutic intranasal doses. Report any unusual or persistent symptoms to your provider.

Who should not use Synapsin + B12?

Not appropriate for patients who are pregnant or breastfeeding. Patients with a history of hormone-sensitive cancers should consult their provider before use given RG3's interaction with estrogen receptor pathways. Those with active nasal infections, significant nasal polyps, or recent nasal surgery should not use intranasal therapy until cleared by their provider. Patients with Leber's hereditary optic neuropathy should avoid high-dose methylcobalamin without specialist evaluation. Not indicated for patients under 18.

Are there any drug interactions?

RG3 has mild acetylcholinesterase inhibitory activity; patients on cholinergic medications including donepezil or rivastigmine should disclose concomitant use as effects may be additive. NAD+ influences sirtuin and PARP pathways; patients on PARP inhibitors or chemotherapy agents should discuss with their oncologist before starting. Methylcobalamin may reduce the efficacy of certain antibiotics including chloramphenicol. Patients on blood thinners should exercise caution given ginseng-derived compounds' mild anticoagulant properties.

Is Synapsin + B12 safe if I have a pre-existing medical condition?

Patients with a history of hormone-sensitive cancers should not use without thorough provider evaluation given RG3's estrogen receptor interaction. Those with neurodegenerative conditions including Alzheimer's or Parkinson's disease may be candidates but require close provider supervision. Patients with pernicious anemia or documented B12 malabsorption may benefit particularly from the intranasal B12 route. Patients with chronic nasal conditions, autoimmune disorders, or significant hepatic impairment require careful provider review before starting.
