Sleep Formula Ultimate is a compounded prescription sublingual troche combining three complementary sleep actives in one nightly dose. It targets the three most common reasons people cannot fall or stay asleep: racing thoughts, physical anxiety, and disrupted circadian rhythm. It dissolves under the tongue before bed and is not a benzodiazepine or habit-forming sleep medication.
Adults who struggle to fall asleep, wake frequently through the night, or feel unrestored in the morning despite hours in bed. It is particularly suited to patients whose sleep issues are driven by anxiety, stress, or a disrupted sleep-wake cycle, and those who have not found adequate relief from over-the-counter melatonin or antihistamine products alone. Provider evaluation is required before prescribing.
Yes. Trazodone and hydroxyzine are prescription-only medications. A valid prescription from a licensed provider is required. Doses are listed on your order form.
Drowsiness, dry mouth, and mild dizziness are the most common effects, which is expected for a sleep formula. Trazodone can rarely cause priapism (prolonged erection) in men and requires immediate medical attention if it occurs. Next-day drowsiness is possible if taken too late or at higher doses.
Not for patients taking MAO inhibitors, as trazodone combined with MAOIs carries a risk of serotonin syndrome. Not recommended during pregnancy or breastfeeding. Patients with cardiac arrhythmia should discuss trazodone's mild QT-prolonging potential with their provider before starting.
Yes. Avoid combining Sleep Formula Ultimate with alcohol, benzodiazepines, opioids, or other CNS depressants due to additive sedation. Trazodone may interact with other serotonergic medications including antidepressants (SSRIs, SNRIs, MAOIs) — combining with MAOIs specifically carries a risk of serotonin syndrome and is contraindicated. Hydroxyzine and trazodone both have mild QT-prolonging potential — patients on other QT-prolonging medications should disclose this to their provider before starting. Disclose all current medications to your provider during your intake.
Patients with cardiac arrhythmia, long QT syndrome, or electrolyte imbalances should discuss this formula with their provider before starting due to the mild QT-prolonging potential of trazodone and hydroxyzine combined. Patients with liver impairment should use caution as both trazodone and hydroxyzine are hepatically metabolized. Not recommended during pregnancy or breastfeeding. Patients with a history of hypotension should be aware that trazodone can cause orthostatic hypotension, particularly in older adults. A provider review is required before prescribing to evaluate your individual health history.
