Overview

Minoxidil + Azelaic Acid + Finasteride + Ketoconazole

Hair Restore Ultra adds azelaic acid to the three-active MFK base. Azelaic acid provides a distinct mechanism of local DHT inhibition through competitive inhibition of 5-alpha reductase that is separate from and additive to finasteride's type II selective blockade. It also reduces scalp inflammation through direct anti-inflammatory activity without the steroid side effect profile of corticosteroids. For patients with seborrheic dermatitis, elevated scalp prostaglandin D2 activity, or incomplete response to finasteride alone, the azelaic acid component provides meaningful additional therapeutic coverage.

Mechanism of Action

Azelaic acid competitively inhibits 5-alpha reductase at both type I and type II isoenzymes through a mechanism distinct from finasteride's binding site, providing additive DHT reduction at the intrafollicular level. It also reduces prostaglandin D2 synthesis, which is elevated in balding scalp and activates CRTH2 receptors on hair follicle stem cells to inhibit hair growth. Ketoconazole clears Malassezia and suppresses scalp-level steroidogenesis. Finasteride blocks type II 5-alpha reductase with high selectivity. Minoxidil maintains anagen through potassium channel-mediated vascular support.

Benefits

Dual-Pathway DHT Inhibition

Azelaic acid and finasteride inhibit 5-alpha reductase through distinct binding mechanisms, providing additive intrafollicular DHT suppression beyond what finasteride alone achieves.

Prostaglandin D2 Suppression

Azelaic acid reduces the elevated PGD2 in balding scalp that activates CRTH2 receptors on follicular stem cells to inhibit hair growth.

Scalp Anti-Inflammatory

Azelaic acid and ketoconazole together reduce both Malassezia-driven and prostaglandin-driven scalp inflammation without corticosteroid side effects.

Anagen Support

Minoxidil maintains the growth phase through perifollicular vasodilation and prostaglandin E2 stimulation to complement the multi-pathway DHT suppression.
