Overview

Minoxidil + Dutasteride + Finasteride +

Tretinoin + Ketoconazole

Hair Restore Extreme combines the most potent topical DHT blockade available with anagen prolongation, penetration enhancement, and antifungal scalp optimization. At 0.1% each, dutasteride and finasteride provide complementary dual-isoenzyme DHT suppression: dutasteride blocks both type I and type II 5-alpha reductase while finasteride adds selective type II coverage. Minoxidil at 5% prolongs anagen and increases perifollicular vasodilation. Tretinoin at 0.01% maximizes co-active scalp penetration without significant irritation. Ketoconazole at 2% addresses Malassezia-driven scalp inflammation and provides supplemental local DHT reduction.

Mechanism of Action

Dutasteride 0.1% inhibits both type I 5-alpha reductase (sebaceous glands, outer root sheath) and type II (follicular dermal papilla), reducing intrafollicular DHT at both enzymatic sites. Finasteride 0.1% adds selective type II inhibition for additive DHT blockade at the primary follicular enzyme. Minoxidil 5% opens ATP-sensitive potassium channels in perifollicular vascular smooth muscle to increase local blood flow and stimulates prostaglandin E2 synthesis in dermal papilla cells to prolong anagen. Tretinoin 0.01% reduces stratum corneum corneocyte cohesion to increase permeability and maximize transdermal delivery of all co-actives into the pilosebaceous unit with minimal retinoid irritation at this low concentration. Ketoconazole 2% inhibits Malassezia furfur colonization, reduces scalp seborrheic inflammation, and exerts weak CYP-mediated inhibition of scalp steroidogenesis to contribute supplemental local DHT reduction.

Benefits

Dual-Isoenzyme DHT Blockade

Dutasteride 0.1% inhibits type I and type II 5-alpha reductase; finasteride 0.1% adds type II coverage for the most complete topical intrafollicular DHT suppression available in one formula.

Anagen Prolongation

Minoxidil 5% extends the hair growth phase through potassium channel-mediated perifollicular vasodilation and PGE2-driven dermal papilla stimulation.

Penetration Enhancement

Tretinoin 0.01% increases transdermal delivery of all co-actives into the pilosebaceous unit without the significant scalp irritation of higher retinoid concentrations.

Scalp Optimization

Ketoconazole 2% reduces Malassezia colonization, suppresses seborrheic scalp inflammation, and provides supplemental local DHT reduction through weak steroidogenic inhibition.

FAQs

Does Hair Restore Extreme require a prescription?

Yes. This formula contains dutasteride, finasteride, and tretinoin, all requiring a valid prescription from a licensed provider.

What are the possible side effects?

Tretinoin 0.01% may cause mild scalp dryness and photosensitivity; daily SPF on exposed scalp is recommended. Minoxidil may cause transient shedding in weeks 4 to 8 and adjacent hypertrichosis. Systemic absorption of dutasteride and finasteride from topical application at these concentrations is substantially lower than oral dosing but should be discussed with your provider.

Who should not use Hair Restore Extreme?

Not for pregnant women or women of childbearing potential due to dutasteride and finasteride teratogenic risk. Not for patients with active scalp infections or known hypersensitivity to any component. Patients with hepatic impairment should discuss the ketoconazole component with their provider.

Are there any drug interactions?

Topical dutasteride and finasteride systemic absorption at these concentrations is low; patients on CYP3A4 inhibitors should disclose all medications to their provider. Tretinoin increases photosensitivity with concurrent retinoid or alpha-hydroxy acid use.

Is Hair Restore Extreme safe if I have a pre-existing medical condition?

Patients with cardiovascular conditions should discuss the minoxidil component with their provider. Patients with hepatic impairment require evaluation for both ketoconazole and dutasteride. PSA levels will be reduced by dutasteride; provider awareness is required for prostate screening interpretation. Not for use in pregnancy under any circumstances.
