Mechanism of Action

Dutasteride inhibits both 5-alpha reductase type I (expressed in sebaceous glands and skin) and type II (expressed in follicular dermal papilla), reducing intrafollicular DHT more completely than finasteride alone. Finasteride provides selective type II inhibition, adding to dutasteride's coverage at the primary follicular enzyme. Minoxidil opens ATP-sensitive potassium channels in perifollicular vascular smooth muscle to increase local blood flow and prolongs anagen by stimulating prostaglandin E2 synthesis in dermal papilla cells. Tretinoin increases stratum corneum permeability by reducing corneocyte cohesion and upregulating follicular epithelial cell differentiation, maximizing the transdermal delivery of all co-actives. Ketoconazole inhibits Malassezia furfur fungal colonization and directly inhibits scalp DHT synthesis through weak CYP-mediated steroidogenic inhibition, while reducing the seborrheic inflammatory microenvironment that worsens androgenetic alopecia.

Does Hair Restore Extreme require a prescription?

Yes. This formula contains dutasteride, finasteride, and tretinoin, all requiring a valid prescription from a licensed provider.

What are the possible side effects?

Tretinoin may cause scalp dryness, peeling, and photosensitivity. Minoxidil may cause transient shedding in weeks 4 to 8 and adjacent hypertrichosis. Systemic absorption of dutasteride and finasteride from topical application is substantially lower than oral dosing but should be discussed with your provider, particularly for women of childbearing potential.

Who should not use Hair Restore Extreme?

Not for pregnant women or women of childbearing potential due to finasteride and dutasteride teratogenic risk. Not for patients with active scalp infections or known hypersensitivity to any component. Ketoconazole requires provider evaluation in patients with hepatic impairment.

Are there any drug interactions?

Topical finasteride and dutasteride systemic absorption is low, but patients on CYP3A4 inhibitors should disclose this to their provider. Tretinoin increases photosensitivity with concurrent alpha-hydroxy acid or additional retinoid use. Ketoconazole at topical scalp concentrations has minimal systemic drug interaction risk.

Is Hair Restore Extreme safe if I have a pre-existing medical condition?

Patients with hepatic impairment should discuss the ketoconazole and dutasteride components with their provider. Patients with cardiovascular conditions should discuss the minoxidil component. Not recommended in pregnancy under any circumstances due to the teratogenic 5-alpha reductase inhibitors.
