Glutathione (GSH) is the most abundant intracellular antioxidant in the human body, present in virtually every cell at millimolar concentrations. It is a tripeptide of glutamate, cysteine, and glycine that functions as the cell's primary redox buffer, xenobiotic conjugator, and immune modulator. Intracellular GSH declines significantly with age, chronic illness, environmental toxin exposure, and oxidative stress. Sublingual troche delivery bypasses GI degradation of oral glutathione, which is largely hydrolyzed by intestinal gamma-glutamyl transpeptidase before systemic absorption, and provides direct mucosal uptake for improved bioavailability.

Mechanism of Action

GSH donates electrons to neutralize reactive oxygen species including hydrogen peroxide, lipid peroxides, and peroxynitrite through glutathione peroxidase (GPx) enzymes. The resulting oxidized glutathione (GSSG) is recycled to GSH by glutathione reductase using NADPH, maintaining the intracellular GSH/GSSG ratio as the primary indicator of cellular redox status. In the liver, GSH conjugates electrophilic xenobiotics and toxic metabolites via glutathione S-transferase (GST) enzymes, producing water-soluble mercapturic acid derivatives for biliary and renal excretion. GSH supports immune function by maintaining the redox status of lymphocytes required for proliferation and cytokine production, and regulates T-helper cell differentiation toward Th1 responses. In melanocytes, GSH inhibits tyrosinase via copper chelation and shifts eumelanin synthesis toward phaeomelanin, producing a skin brightening effect with sustained use.

Who should not use Glutathione Troche?

Not for patients with known hypersensitivity to glutathione or any excipient. Patients with asthma should use with caution as inhaled glutathione has rarely triggered bronchospasm, though sublingual use is lower risk. Use during pregnancy or breastfeeding requires provider evaluation.

Are there any drug interactions?

Glutathione may reduce the efficacy of certain chemotherapy agents that rely on intracellular oxidative stress for cytotoxicity, including platinum-based compounds. Patients undergoing chemotherapy must not use glutathione without oncologist approval. Concurrent nitroglycerin use may produce additive vasodilation.

Is Glutathione Troche safe if I have a pre-existing medical condition?

Patients with active or prior malignancy must discuss with their oncologist before starting given glutathione's cytoprotective effect on rapidly dividing cells.
