Pharmacology

Dutasteride is a dual-isoenzyme 5-alpha reductase inhibitor that blocks both type I and type II enzymes, reducing serum DHT by up to 90 percent. Finasteride inhibits only type II and reduces DHT by approximately 70 percent. This difference is clinically significant in patients with inadequate response to finasteride or more aggressive androgenetic alopecia.

Mechanism of Action

Dutasteride competitively and irreversibly inhibits both type I and type II 5-alpha reductase isoenzymes, preventing testosterone conversion to dihydrotestosterone in scalp follicles, skin, and prostate tissue. The resulting suppression of intrafollicular DHT halts follicular miniaturization and allows existing follicles to return to normal growth cycles.

Dutasteride is metabolized by CYP3A4. Inhibitors including ritonavir, ketoconazole, and verapamil can significantly increase dutasteride blood levels. Patients on these medications require provider evaluation before starting.

Is Dutasteride safe if I have a pre-existing medical condition?

Patients with hepatic impairment require dose reduction and close monitoring. Patients with prostate cancer history require urologist evaluation, as dutasteride reduces PSA levels and can affect monitoring accuracy. Regular PSA and liver function monitoring is recommended.
